The findings of a recent study on the health of older Australians have revealed a striking pattern: multimorbidity is the norm, not the exception. This means that chronic conditions tend to occur in distinct groups, or clusters, rather than at random. The study, published in the Medical Journal of Australia, analyzed the health records of over 4.4 million Australians aged 65 and over, and found that 76% of them have two or more chronic conditions, with about one-third having five or more. What makes this particularly fascinating is that these conditions tend to cluster together in three key areas: cardiovascular-metabolic, neuropsychiatric-functional decline, and inflammatory-musculoskeletal-cancer. This clustering pattern is not just an interesting observation; it has significant implications for healthcare planning and coordination. From my perspective, the study highlights the need for a more holistic approach to healthcare, one that takes into account the interconnectedness of these conditions. The current healthcare system, which rewards episodic, single-problem care, is not equipped to handle the complex needs of older patients with multiple chronic conditions. In my opinion, the study's findings underscore the importance of general practice in coordinating care for older patients. GPs and primary care practices are best placed to plan and coordinate care for these patients, as they are the only clinicians who see the whole person rather than just a single organ system. However, the current MBS structure does not adequately reward this role, and GPs are often left to manage the fragmentation of care on their own. This raises a deeper question: how can we better support GPs in addressing multimorbidity in older patients? One thing that immediately stands out is the need for sustainable funding for longer consultations, as well as for practice-embedded nursing and care coordination. Managing five or more chronic conditions cannot be done well in short appointments, and the current chronic disease item structures do not reflect the cognitive work involved. In my view, the study's findings also highlight the need for a more equitable approach to healthcare. The figures clearly reveal the equity disparity, with all three clusters being most prevalent in the most disadvantaged areas of Australia. This is a critical issue that needs to be addressed, as multimorbidity is not evenly distributed, and neither should our response be. In conclusion, the study's findings have significant implications for healthcare planning and coordination, and they underscore the need for a more holistic and equitable approach to caring for older patients with multiple chronic conditions. It is my hope that these findings will lead to a more coordinated and supportive healthcare system for the aging population, one that takes into account the complex needs of these patients and provides them with the care they deserve.
